When answering questions, we will collect technical information relating to the usage of the tools such as your IP address, operating system, device, features used, content viewed and downloaded, the dates and times of your interactions with the surveys and other information.

Risks and Benefits
The risks associated with participating in Thrivable are the loss of confidentiality of data. The benefits which may reasonably be expected to result from this study are improvements to the products and services provided by commercial organizations, non-profit organizations, and government related to the questions that you answer. We cannot and do not guarantee or promise that you will receive any benefits from this study. Your decision whether or not to participate in this study will not affect your employment/medical care.
Time Involvement
Each survey will begin with an estimate of how long it will take to complete.
Payments/Reimbursement
If reimbursement is available, it will be noted at the beginning of any research activity.
Participant’s Rights
If you have read this policy and have decided to participate in this project, please understand your participation is voluntary and you have the right to withdraw your consent or discontinue participation at any time without penalty or loss of benefits to which you are otherwise entitled. If you withdraw from the Study, we will retain all of the information about you that was collected prior to that point and continue to use such information.
The results of this research study may be presented at scientific or professional meetings or published in scientific journals. However, your identity will not be disclosed. Your identity and/or your personal health information will not be disclosed except as authorized by you or as required by law. However, there is always some risk that even de-identified information might be re-identified. You have the right to refuse to answer particular questions. Identifiers might be removed from identifiable private information and, after such removal, the information could be used for future research studies or distributed to another investigator for future research studies without additional informed consent from you.
Patient information may be provided to Federal and other regulatory agencies as required. The Food and Drug Administration (FDA), for example, may inspect research records and learn your identity if this study falls within its jurisdiction.
